  I have been a patient of Maple Street Clinic for several years, I have seen a genuine degradation in the quality of care over the last couple of years. There is an extreme lack of professional abilities by the nurses aids to cross reference records and follow procedures regarding needed medications, forwarding records and giving proper shots. Our family alone has had several very worrisome situations with being given the wrong shots or medicine we are allergic to. In the past they have always been very attentive to the needs of the patients but this has also taken a negative turn, where we used to be notified of needed preventative tests and appointments, we now have to initiate the calls ourselves to see if we are due for these services. I realize that the medical field is quite complicated but I think it is irresponsible of a medical practice to let it's standards slip so dramatically. I value my life and feel that maintaining my business with this clinic will be very detrimental to my health.
